Listed below are some common situations that result in change of ownership or a change to owner accounts. Below each situation you will find the required documentation to effect the change. (Occasionally, some situations will require additional documentation.) All change requests must be submitted in writing with the appropriate documentation:

Q: Name Change (due to Marriage, Divorce, etc.)

A: One of the following: Marriage Certificate Divorce Decree with Name Reinstatement other document(s) that supports the name change

Q: Address Change

A: Submit request in writing with signature. (see FAQ's)

Q: Bank Change

A: Submit request in writing with EFT Authorization Form.

Q: Company or Corporation name change

A: Certificate of Name Change, including tax ID

Q: Company or Corporation merger

A: Certificate of Merger, including tax ID

Q: Appointment of Attorney-in-Fact, Trustee, POA, etc.

A: One of the following documents: Copy of Power of Attorney Instrument appointing trustee Other document(s) supporting appointed power

Q: Trust/Partnership Created

A: Trust or Partnership Agreement and, if applicable, recorded document conveying interest in property to the trust or partnership.

Q: Sale of Interest

A: Conveyance filed of record in the county where the property is located

Q: Owner distributes interest to heirs through conveyance (Mineral Deed)

A: Copy of recorded Deed.

Q: Member of Joint Tenancy dies

A: Death Certificate of Joint Tenant

Q: Owner died with a Will (testate) and Probate proceedings to be conducted in state where the property is located.

A: All of the following documents: Death Certificate Order Admitting Will to Probate Last Will and Testament Letters Testamentary Final Decree of Distribution

Q: Owner died with a Will (testate) and Probate proceedings to be conducted in a state other than the state where the property is located.

A: All of the following documents and information: Death Certificate Order Admitting Will to Probate Last Will and Testament Letters Testamentary Documentation of ancillary probate proceedings Affidavit of Heirship (recorded in county of property) Names and addressed of all legal heirs

Q: Owner died without a Will (intestate) (Laws of Descent and Distribution will be applied)

A: All of the following documents and information: Death Certificate Letters of Administration Affidavit of Heirship (recorded in county property) Names and addresses of all legal heirs

* All changes of ownership must be filed in the County or Parish where property is located.
